Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient care in home and community settings. You will conduct comprehensive assessments, develop and adjust care plans, and collaborate with an interdisciplinary team to ensure compassionate, patient-centered end-of-life care.

Responsibilities include delivering hands-on nursing care, educating families, and documenting care activities while maintaining regulatory compliance.

Qualifications

• Active RN license in the state of employment.
• Hospice or palliative care experience preferred.
• Ability to travel to patient homes.
• Strong communication with patients and families.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per care plans.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across interdisciplinary teams and consult with physicians.
• Educate patients, families, and caregivers on disease, end-of-life care, and medications.
