Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to provide in-home nursing care and coordinate patient plans of care. You will assess, educate families, and collaborate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care in patients' homes or alternative settings.

The role requires an active RN license and at least 1 year of hospice or home health experience, with CPR certification and a valid driver's license.

Qualifications

• Active RN license in the state of employment.
• 1 year of RN experience in hospice, palliative care, home health, or related setting preferred.
• CPR certification & valid driver's license.

Responsibilities

• Perform comprehensive patient assessments and ongoing reassessments in patients' homes or care settings.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency and risk identification.
• Coordinate care with IDG including physicians, social workers, and other team members.
