Job summary

Gentiva seeks a compassionate Hospice RN / RN Case Manager to lead patient assessments, deliver skilled nursing interventions, and develop personalized care plans in home or community settings. You will collaborate with physicians, LPNs, CNAs, social workers, and chaplains to ensure holistic, patient-centered care.

CPR-certified, licensed in-state, and 1+ year hospice/home health experience preferred. Join a team that supports patients and families with dignity and comfort, providing education

Qualifications

• Minimum 1 year RN experience in hospice, home health, oncology, or related settings.
• Current RN license in the state of employment.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education to patients, family members and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Educate patients and caregivers regarding disease, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and communicate changes in condition to the care team.
