Job summary

Gentiva Hospice is seeking a dedicated Registered Nurse (RN) Case Manager to lead in-home hospice care. You will perform comprehensive patient assessments, deliver skilled nursing care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, patient-centered plans.

Responsibilities include educating families, maintaining clinical documentation, and participating in quality improvement initiatives while adhering to regulatory standards.

Qualifications

• RN licensure required in the state of employment.
• Hospice/palliative care experience preferred.
• Minimum 1 year as an RN in relevant settings such as hospice, home health, or acute care.

Responsibilities

• Be the primary contact for patients and families, guiding them through hospice care.
• Deliver and document skilled nursing care based on individualized Plans of Care (home, assisted living, etc).
• Perform initial and ongoing assessments and adjust care as needed.
• Collaborate with IDT members to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families; educate caregivers.
• Document timely clinical records and maintain compliance with infection control.
