Job summary

Gentiva is seeking a dedicated Hospice RN / Case Manager to lead patient care in home environments, coordinating with physicians, social workers, chaplains, and other team members to deliver compassionate, personalized end-of-life care. You will assess, plan, and document care, while educating patients and families and ensuring timely interventions.

The role requires an active RN license in the state of employment and at least 1 year of RN experience (hospice experience preferred), CPR

Qualifications

• RN with current license in the state of employment.
• Minimum of 1 year RN experience in acute care, hospice or home health; hospice/palliative care preferred.
• CPR Certification.
• Current automobile insurance and valid driver’s license.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education to patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including patient visit frequency to meet needs and risk.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide patient and caregiver education regarding disease, self-care, end-of-life care, medications, nutrition and dietary needs.
• Communicate changes in condition to appropriate provider.
• Support continuity of care by coordinating referrals and transitions in care levels.
• Document timely clinical records including orders, progress notes, and care plans.
• Maintain effective working relationships with IDG and involve team as needed.
• Participate in on-call and patient support activities.
