Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to guide patients and families through the hospice journey, delivering hands‑on nursing care in home or community settings. You will develop personalized care plans, reassess patient needs, and coordinate with physicians, CNAs, and social workers to ensure compassionate, comprehensive care.

Qualifications

• Minimum of one year RN experience in hospice, home health, or related field.
• Hospice or palliative care experience preferred.
• Current RN license in the state of employment.
• CPR Certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in‑person/telephonic RN assessments and educate patients and caregivers as appropriate.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across IDG with physicians and other team members.
• Educate patients and caregivers on disease process, end‑of‑life care, medications, nutrition, and self‑care.
