Job summary

Gentiva Hospice is seeking a compassionate Hospice RN / RN Case Manager to lead patient-centered end‑of‑life care in patients' homes or equivalent settings. You will guide families, perform comprehensive assessments, administer skilled nursing care, and coordinate with physicians, social workers, and chaplains to implement personalized plans of care.

The role emphasizes education for patients and caregivers, symptom management, timely documentation, and adherence to regulatory standards.

Qualifications

• Active RN license in Rhode Island
• CPR certification
• Hospice or home health experience preferred

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telecommunication, and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care, including visit frequency and risk identification.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Provide education on disease processes, self-care, end-of-life care, medications, nutrition, and dietary needs.
• Communicate changes in condition and signs of decline to the appropriate provider.
• Support continuity of care by recognizing and supporting referrals and transitions in care levels.
• Document timely clinical records, screening results, orders, progress notes, and care plans.
• Maintain relationships with the IDG and involve the team when care plans warrant collaboration.
• Participate in on-call and patient support activities, delivering on-call services competently.
