Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver in-home nursing care and oversee comprehensive care plans for patients with life-limiting illness. You will guide families, collaborate with an interdisciplinary team, and ensure safe, compassionate care in patients’ homes or settings they call home.

The role emphasizes education, symptom management, and bereavement support, with emphasis on documentation and regulatory compliance. A current NC/SC license and CPR are required.

Qualifications

• RN with current license to practice in the state of employment.
• Minimum 1 year RN experience in hospice, home health, or related settings.
• CPR certification.

Responsibilities

• Perform comprehensive patient assessments and reassessments, educate patients and caregivers.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized care plans and visit frequency.
• Coordinate care across interdisciplinary teams and physicians.
• Educate patients and caregivers on disease, self-care, medications, and end-of-life care.
• Communicate changes in condition to the health team; ensure timely interventions.
• Document timely clinical records and maintain regulatory compliance.
• Participate in on-call and patient support activities.
