Job summary

Gentiva Hospice is seeking a skilled RN to serve as RN Case Manager, delivering compassionate, in-home hospice and palliative care. You will perform initial and ongoing assessments, develop care plans, and coordinate with an interdisciplinary team to meet patient goals.

This role emphasizes education for families, symptom management, and timely documentation, with on-call duties and compliance with hospice standards.

Qualifications

• Valid RN license in the state of employment.
• Minimum of 1 year of RN experience in hospice, home health, or related field.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing assessments.
• Deliver and document skilled nursing care per Plan of Care, in home or facility.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care with physicians, LPNs, CNAs, social workers, and chaplains.
• Educate patients and caregivers on disease, end-of-life care, medications, nutrition.
• Document timely clinical records and maintain compliance.
• Participate in on-call and patient support activities.
