Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct home-based hospice care, drawing on extensive nursing experience and community health education. The professional nurse builds from community resources to meet the needs of patients and families in homes and communities.

The RN conducts comprehensive assessments, develops plans of care, administers medications, and educates patients and families.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license.
• Minimum of two (2) years’ experience, at least one in public health, home care, or hospice nursing is preferred.

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Initiates and revises plan of care as patient status changes. Administers medications and treatments as prescribed.
• Counsels patient and family, provides health instructions, and supports self-care and comfort care.
• Acts as Case Manager when assigned and coordinates patient care for caseload.
• Participates in on-call duties and coordinates equipment and services.
• Supervises ancillary personnel and participates in continuing education and QA activities.

Public health/home care experience (preferred)
