Job summary

This role requires an Ohio RN license, 1 year acute care or medical-surgical nursing experience, and proficiency with Windows and EMR systems.

Qualifications

• Current Ohio RN license to practice.
• One year experience in acute care or medical-surgical nursing.
• Hospice experience is preferred.
• Proficiency with Windows, Microsoft Office, and the Internet.

Responsibilities

• Provide ongoing assessment and nursing care to patients in homes and facilities, and develop a comprehensive care plan.
• Follow nursing standards and hospice protocols to ensure quality care.
• Provide symptom management and communicate changes in patient condition to other providers.
• Oversee and evaluate the patient’s Plan of Care and its effectiveness.
• Prepare for IDG by reviewing Plan of Care, reconciling medications, and projecting needs.
