Job summary

Qualifications

• Registered nurse licensed in the state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in acute care, hospice, home health, or related fields.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits, with education for patients, families and caregivers.
• Deliver nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and nurses.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications, nutrition and dietary needs.
• Communicate condition changes to appropriate providers and support transitions in levels of care.
• Document timely clinical records and maintain infection control and compliance.
• Support quality improvement and scheduling initiatives to ensure compassionate, consistent care.
