Job summary

Gentiva is seeking a compassionate Hospice RN / Case Manager to deliver personalized nursing care in patients' homes or other call sites. You will assess, plan, implement, and document care while guiding families through hospice decisions and transitions.

The role emphasizes collaboration with physicians, LPNs, CNAs, social workers, and chaplains, with a focus on comfort, dignity, and education for patients and loved ones.

Qualifications

• Active RN license in the state of employment
• Hospice or palliative care experience preferred
• Excellent patient assessment and communication skills
• Ability to perform home visits and document care plans

Responsibilities

• Perform comprehensive patient assessments and reassessments both in person and by phone
• Deliver and document skilled nursing interventions and symptom management
• Develop individualized care plans with visit frequency
• Coordinate care with IDG and involved physicians/nurses
• Provide education to patients and caregivers on disease, medications, and end-of-life care
• Maintain timely clinical documentation and ensure compliance with standards
• Participate in on-call coverage as required
