Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to deliver personalized, in-home hospice nursing care. You will assess patients, collaborate with an interdisciplinary team, educate families, and ensure accurate documentation and care plans are updated.

This full-time, remote role serves patients in home settings across the region, with focus on compassionate end-of-life care. Ideal candidates have at least 1 year of RN experience in hospice or related fields, with licensure in the employment

Qualifications

• RN licensure in the state of employment.
• Minimum 1 year RN experience in hospice, home health, oncology, or related areas.
• Preferred hospice or palliative care experience.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management.
• Develop and update care plans with visit frequency.
• Coordinate care with physicians and IDG.
• Educate patients and caregivers on disease, end-of-life care, and medications.
