Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to provide in-home nursing care, assessments, and ongoing care coordination for patients facing life-limiting illness. You will collaborate with an interdisciplinary team to refine care plans, educate families, and ensure compassionate, high-quality end-of-life support.

The role emphasizes thorough documentation, adherence to infection control, and on-call support to assist patients and families in navigating hospice services.

Qualifications

• RN with current licensure in the state of employment.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and ongoing re-assessments in patient homes or care settings.
• Deliver and document skilled nursing interventions and symptom management.
• Develop and update individualized care plans with visit frequencies to meet patient needs.
• Coordinate care with physicians, LPNs, CNAs, social workers and chaplains.
• Educate patients and caregivers on disease processes, end-of-life care, and medication management.
