Job summary

Gentiva Hospice is seeking an experienced RN Case Manager to oversee direct patient care in home-based hospice settings. You will assess patients, manage symptoms, and educate families while coordinating with physicians, social workers, and other team members.

This role emphasizes interdisciplinary collaboration, documentation, and quality of life improvements for patients and their families. Flex/Per Diem opportunities are available in a supportive, compliant environment.

Qualifications

• Active RN license in state of employment.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in acute care, hospice, home health, or related field.

Responsibilities

• Perform comprehensive patient assessments in home or facility settings.
• Deliver nursing interventions and symptom management per care plans.
• Coordinate care with IDG members and update plans of care.
