Job summary

Heartland Hospice is seeking a dedicated RN Case Manager to lead in-home hospice care. You will perform detailed patient assessments, implement and adjust plans of care, and coordinate with physicians, LPNs, CNAs, social workers, and chaplains to ensure compassionate, comprehensive end-of-life support.

Strong communication and regulatory compliance are essential. One year of RN experience in hospice or related fields is preferred, with a current nursing license and CPR certification.

Qualifications

• Registered Nurse with current license to practice in the state of employment.
• Minimum of one year RN experience in acute care, oncology, hospice or home health.
• Hospice or palliative care experience preferred.
• CPR Certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments in person or via telephonic visits.
• Deliver skilled nursing interventions and symptom management per plan of care.
• Develop and update individualized plans of care with visit frequency.
• Coordinate care across interdisciplinary teams including physicians.
• Educate patients and caregivers on disease progression, medications, and at-home care.
• Document timely clinical records and communicate changes to the care team.
