Job summary

Heartland Hospice is seeking an experienced Hospice RN / RN Case Manager to lead patient care in home settings. You will perform comprehensive RN assessments, develop individualized plans of care, and coordinate interdisciplinary team efforts to support patients and families with dignity and comfort.

The role emphasizes education, symptom management, and timely documentation, with on-call responsibilities as needed to ensure seamless continuity of care across services.

Qualifications

• RN licensure in good standing.
• Hospice or palliative care experience preferred.
• Minimum 1 year RN experience in related settings.

Responsibilities

• Perform comprehensive RN assessments and educate patients, families, and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency.
• Coordinate care across interdisciplinary teams including physicians and nurse practitioners.
• Educate patients and caregivers on disease, self-care, medications, and nutrition.
• Communicate change in condition or signs of decline to providers.
• Support continuity of care by recognizing referrals and transitions in levels of care.
• Document timely clinical records including screening findings, orders, progress notes, and care plans.
• Maintain effective relationships with IDG and involve the team when the care plan warrants.
• Participate in on-call and patient support activities, providing responsive coverage.
