Job summary

Heartland Hospice is seeking an experienced Hospice RN / RN Case Manager to deliver compassionate, patient-centered care in patients' homes or facilities. You will assess, plan, implement and document care, collaborate with an interdisciplinary team, educate families, and support patients through end-of-life care.

A valid RN license and CPR are required, with at least one year of hospice or home health experience. Sign-on bonus of up to $5,000 is available.

Qualifications

• RN license in the state of employment
• 1 year RN experience in hospice/home health (preferred)
• CPR certification required

Responsibilities

• Be the primary point of contact for patients and families.
• Deliver nursing care per Plan of Care in home or facility settings.
• Perform initial and ongoing patient assessments and adjust care.
• Collaborate with IDG to refine care plans.
• Provide emotional, physical, and spiritual support to patients and families.
• Educate caregivers on home care and safety.
• Communicate patient updates to the care team promptly.
• Maintain accurate clinical documentation and infection control.
