Job summary

Heartland Hospice is seeking a dedicated Registered Nurse to join our team as a Hospice RN / RN Case Manager. You will lead in-home assessments, deliver skilled nursing care, and coordinate with physicians and an interdisciplinary team to ensure compassionate, high-quality end-of-life care for patients and families.

Qualifications

• Registered Nurse licensure in state of employment
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice or home health
• Hospice or palliative care experience preferred
• CPR certification required
• Current automobile insurance and valid driver’s license

Responsibilities

• Perform comprehensive patient assessments and reassessments, provide education to patients, family members and caregivers
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care and determine visit frequency
• Coordinate care with physicians and interdisciplinary team members
• Educate patients and caregivers on disease processes, self-care, end-of-life care, medications and nutrition
• Communicate changes in patient condition to care team for timely interventions
• Maintain timely clinical documentation and comply with infection control standards
• Support continuity of care across home health, palliative and hospice services
• Participate in on-call and patient support activities as applicable
