Job summary

Heartland Hospice in Ames, IA is seeking an experienced Hospice RN / RN Case Manager to lead patient care in home settings and collaborate with an interdisciplinary team. You will perform comprehensive assessments, develop plans of care, educate families, and ensure high-quality symptom management in alignment with goals of care.

The role requires an active RN license, CPR certification, and a minimum of 1 year of RN experience, with hospice/home health experience preferred.

Qualifications

• Current license to practice nursing in the state of requested employment.
• 1 year of RN experience minimum; hospice or palliative care experience preferred.
• CPR certification required.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person and telephonic visits; provide education to patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized plans of care with visit frequency and risk identification.
• Coordinate care across interdisciplinary teams and consult with physicians and other providers.
• Educate patients and caregivers about disease, self-care, end-of-life care, medications and nutrition.
• Document timely clinical records and maintain compliance with infection control standards.
• Support on-call and patient support activities as applicable.
