Job summary

Heartland Hospice seeks a dedicated Hospice RN / RN Case Manager to lead in-home patient care, perform assessments, and implement plans of care. You will work with an interdisciplinary team to deliver compassionate hospice services and educate families in end-of-life care within patients' homes.

The role emphasizes patient-centered communication, clinical excellence, and adherence to regulatory standards while supporting bereavement guidance and education for families.

Qualifications

• Must hold a current nursing license in the state of employment.
• Hospice or palliative care experience preferred.
• Experience in home health or hospice settings is desirable.

Responsibilities

• Perform comprehensive patient assessments and reassessments, including in-person or telephonic RN assessments.
• Deliver skilled nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans and determine visit frequency.
• Coordinate care with IDG and physicians to refine plans of care.
• Educate patients and caregivers about disease, self-care, and end-of-life considerations.
• Document clinical records timely and accurately; maintain infection control standards.
• Support continuity of care across hospice, palliative, and home health referrals.
