Job summary

Advocate Health Hospice Lake Zurich is seeking a dedicated Home Health Aide to provide compassionate care under the supervision of an RN. The role is full-time, Monday–Friday 8:00 am–4:30 pm with every 7th weekend on-call from 8 am to 5 pm and holidays on-call rotation.

Responsibilities include assisting with personal care, monitoring patient condition, and documenting care while upholding HIPAA and privacy standards.

Qualifications

• Nurse assistant certification in the applicable state.
• BLS for Healthcare Providers certification.
• Valid driver’s license required.

Responsibilities

• Contributes to effective patient outcomes and provides competent patient care as assigned by the RN and plan of care.
• Assists with personal care, feeding, hygiene, mobility, and daily living tasks as directed by the RN.
• Documents care provided and monitors patient condition, reporting changes to RN or supervisor per standards.
• Demonstrates professionalism, teamwork, and flexibility in scheduling to support department needs.
• Adheres to regulatory requirements and maintains required annual in-service training hours.
