Job summary

Heart N Home Hospice, a part of LHC Group and within the Optum family, is seeking a Home and Community Based Services Aide to support a client in their home. The role focuses on personal care and daily living activities to help the client remain at home.

Responsibilities include bathing, grooming, meal prep and accompanying appointments, while documenting conditions with EVV, ensuring safety and appropriate response to needs. CPR preferred; flexible hours and independence valued.

Qualifications

• Current CPR certification or ability to complete within 90 days of hire.
• 6+ months of home care experience.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
