Advocate Health seeks a Home Health Aide for the Green Bay hospice team. The role offers a 40-hour work week, Monday–Friday, 8:00 a.m.–4:30 p.m., with travel to neighboring territories as needed.

Requirements include HHA certification in the state, BLS, a valid driver's license, and ability to lift up to 35 pounds. The position emphasizes compassionate patient care within a hospice setting.

Qualifications

• Must be certified as a nurse assistant in the state where the teammate practices.
• Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices.
• Basic Life Support (BLS) for Healthcare Providers certification by the American Heart Association (AHA) must be obtained within 6 months.
• A valid driver’s license issued by the Division of Motor Vehicles.

Responsabilités

• Contributes to effective patient outcomes and provides competent patient care as assigned by RN and plan of care.
• Performs personal care tasks (feeding, laundry, light room cleaning) and patient safety measures under RN guidance.
• Documents patient information and health changes per agency and regulatory standards; upholds HIPAA/privacy rules.
• Maintains professionalism and cooperates with team, with scheduling flexibility and commitment to patient, team, and organization.
• Adheres to regulatory requirements and completes required annual in-service and hospice education hours.
