Job summary

Advocate Aurora Hospice is seeking a dedicated Home Health Aide to support terminally ill patients in their homes. You will assist with daily living activities, monitor vital signs, and report changes to the RN or manager while adhering to HIPAA and hospice standards.

Qualifications

• Must be certified as a nurse assistant in the state in which the teammate practices.
• Maintains patient confidentiality per HIPAA and privacy rules.
• Completes required annual training and licensing requirements.

Responsibilities

• Provide competent patient care under RN supervision and plan of care.
• Perform personal care needs, monitor vitals, and support daily living activities.
• Document care accurately and timely in accordance with agency standards.
• Communicate changes in patient condition to RN or manager.
• Maintain professionalism and cooperate with team members.
