Job summary

Lakeview is looking to hire a Home Health Aide to join our Homecare/Hospice team. You will assist professional personnel to provide patient care according to department and hospital standards, and perform supportive tasks under the supervision of a Case Manager.

Responsibilities include taking vital signs, assisting with ADLs, observing safety in the home, documenting in the EHR, and driving to client residences.

Qualifications

• Must be 18 or older and have at least 1 year of driving experience.
• Must have a MN or WI CNA certificate at start date.
• Current BLS CPR/AED for Professional Rescuer & Health Care Provider required.

Responsibilities

• Assist professional personnel to provide patient care according to department standards.
• Perform tasks such as vital signs, ADLs, transfers, and basic wound care as assigned.
• Document observations and changes in patient condition to the Case Manager.
• Drive to client residences to provide in-home care.
