Job summary

At Home Hospice, part of LHC Group under UnitedHealth Group, seeks a Home Health Aide to support patients at home. You will provide personal care and assist with daily living activities to help individuals remain in their own homes.

Responsibilities include bathing, grooming, meal prep, appointments, and monitoring client conditions with EVV documentation. Flexible hours and a supportive team culture are emphasized.

Qualifications

• Must have valid driver's license, vehicle insurance, and reliable transportation or access to public transportation.
• Home Health Aide: minimum 1 year full-time direct client care experience in hospital or nursing facility, or 1 year within last 5 years in an agency setting, or validated home health training.
• Nursing student with department/ dean documentation demonstrating competency in basic nursing skills.
• Must NOT be listed on Texas DHS Nurse Aide Registry with any misconduct findings.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal prep, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment, and respond promptly to client needs.
• Ability to work with diverse client backgrounds regardless of age, race, creed, color, sex, or disability.
