Job summary

At Home Hospice, part of LHC Group within the Optum family, is seeking a Home and Community Based Services Aide to support clients at home, assisting with personal hygiene and daily living activities to help them remain in their own homes.

Responsibilities include personal care, meal preparation, accompany clients to appointments, monitoring vital signs, documenting conditions, and ensuring a safe environment.

Qualifications

• Requires current driver’s license and reliable transportation.
• Preferred CPR certification.
• Flexible hours and ability to work independently.

Responsibilities

• Provide personal care and assist with daily living activities.
• Support household tasks, meal preparation, and accompany clients to appointments.
• Monitor and document client condition and vital signs; use EVV system.
