Job summary

Advocate Aurora Hospice in Illinois seeks a compassionate home health aide to support terminally ill patients under RN supervision. You will assist with personal care, monitor vitals, and document care while upholding HIPAA and confidentiality.

Travel within the service area is required, and flexibility in scheduling is expected. Required CNA certification, active state registry, and BLS (AHA) within 6 months; a valid driver’s license is also necessary.

Qualifications

• Must be certified as a Nurse Assistant in the state where you practice.
• Nurse Assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health/hospice.
• BLS for Healthcare Providers certification (AHA) to be obtained within 6 months.
• Must have a valid driver’s license.

Responsibilities

• Contributes to effective patient outcomes and provides competent care under the RN and plan of care.
• Performs personal care tasks (feeding, laundry, room care) and monitors patient safety.
• Maintains documentation per agency and regulatory standards; preserves patient confidentiality (HIPAA).
• Demonstrates professionalism, supports team goals, and participates in CQI projects.
• Adheres to regulatory requirements and hospice service standards.
