Advocate Aurora Hospice is seeking a Home Health Aide to provide compassionate, in-home care under RN supervision, assisting with daily living activities, and supporting comfort for patients in their homes.

The role requires a current CNA certification, up-to-date BLS, and a valid driver’s license. You will document care, maintain patient confidentiality, and participate in quality improvement activities within the hospice program.

Formación

• Must be certified as a nurse assistant in the state where the teammate practices.
• Nurse assistant in good standing with the State Nurse Aide Registry with eligibility to work in home health agencies and hospices.
• BLS for Healthcare Providers certification issued by the American Heart Association (AHA) must be obtained within 6 months.
• A valid driver’s license issued by the state.
• No experience required.

Responsabilidades

• Contributes to effective patient outcomes and provides competent care assigned by RN and plan of care.
• Reports changes in patient condition to RN or manager and observes hospice philosophy.
• Performs personal care tasks including feeding, hygiene, and bed linen changes; ensures patient safety.
• Maintains documentation per agency and state requirements while protecting patient confidentiality (HIPAA).
• Demonstrates professionalism, supports team goals, and participates in CQI and quality improvement projects.
