Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to support patients under the supervision of a registered nurse, performing personal care and comfort tasks per the care plan.

You will document observations, report changes to the RN Case Manager, and assist with activities of daily living, transfers, and light housekeeping while promoting dignity and safety in a home or facility setting.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfortable caring for the terminally ill.
• Prior hospice, orderly, acute, rehabilitative, or long-term care facility experience; 1 year home health/hospice preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide patient and family with positive communication and maintain confidentiality.
• Ensure safety and proper use of equipment; assist with transfers and mobility.
• Assist with bathing, grooming, oral hygiene, and changing bed linens.
• Help with dressing, feeding, and overall daily living activities.
• Report pain or changes to the RN Case Manager and document observations.
• Observe skin condition and prevent tissue breakdown; ensure clean living area.
• Assist with light housekeeping and medications reminders as allowed by state rules.
• Document care in EMR and participate in care planning with the team.
• Support respite care and maintain respectful, ethical care environment.
