Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to work under the supervision of a registered nurse and perform patient care tasks to meet the plan of care and promote comfort. Qualifications

• Completion of a state-approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC).
• Current Healthcare Provider CPR certification (hands-on demonstration required).
• Current Food Handlers Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Prior hospice, home health, or long-term care experience preferred.
• Licensed driver with reliable vehicle and valid insurance.

Responsibilities

• Provide personal care such as bathing, back rubs, oral hygiene, and changing bed linens as assigned.
• Assist with dressing, undressing, and feeding the patient.
• Monitor patient safety and use equipment correctly (rails, O2, etc.).
• Report pain, changes in condition, or observations to the RN Case Manager.
• Assist in ambulation and transfers and provide range of motion as directed.

Education

Certified Nursing Assistant (CNA)

CPR Certification

Food Handlers Permit

High School Diploma or GED

Ability to Read and Follow Instructions

Hospice/Home Health Experience
