Job summary

Qualifications

Certified Nursing Assistant (CNA) certificate or equivalent training.
Current CPR certification as required by state.
Food Handler's Permit.
High school diploma or GED.
NAC license or certification.

Responsibilities

Perform personal care tasks as assigned and report to the RN Case Manager.
Provide compassionate communication with patients and families, maintaining confidentiality.
Ensure safety of patient and use equipment properly.
Assist with baths, grooming, feeding, and mobility as assigned.
Document care and observations in EMR and care plans.
Report changes in patient condition to the RN Case Manager promptly.
Keep living area clean and organized and manage light housekeeping duties.
