Job summary

Affinity Hospice Management LLC is seeking a compassionate hospice aide to support patients under RN supervision. You will perform personal care, monitor conditions, document care, and assist families with respectful communication.

Must have CNA/NAC, CPR, Food Handlers, and a HS diploma or GED. Prior hospice or long-term care experience is preferred; familiarity with EMR is a plus. Driving to patient homes may be required.

Qualifications

• Completion of state-approved CNA course or equivalent training.
• NAC certification is required.
• Current CPR certification (hands-on preferred).
• Food Handlers Permit is required.
• High school diploma or GED required.
• Ability to read and follow written instructions and document care.
• Comfort with hospice philosophy and terminally ill patients.
• 1 year of hospice or long-term care experience preferred.
• Basic computer skills to document in EMR.
• 18 years of age or older.
• Licensed driver with insured vehicle in good working order.

Responsibilities

• Provide personal care and assist with daily activities as assigned.
• Communicate with patient families with respect and confidentiality.
• Ensure patient safety and proper use of equipment.
• Assist with baths, dressing, feeding, and hygiene as needed.
• Report pain or changes to RN Case Manager promptly.
• Assist with mobility, ROM, light housekeeping and laundry.
• Support medication reminders where allowed by state rules.
• Maintain clean living area and adhere to care protocols.
• Participate in QA and performance improvement activities.
• Respect patient and family environment and privacy.
• Report any neglect or abuse as required by law.
