Job summary

Affinity Hospice Management LLC seeks a compassionate hospice aide to work under the supervision of a registered nurse. You will perform tasks outlined in the patient’s plan of care to meet personal needs and promote comfort, while diligently reporting observations and care performed.

You will be assigned to ensure quality, continuity and safety of patient care, collaborating with the RN Case Manager and family as appropriate.

Qualifications

• Certified Nursing Assistant (CNA) training completed.
• CPR certification current and hands-on (not internet-based).
• Food Handler Permit issued.
• High school diploma or GED or equivalent.
• Ability to read and follow written instructions and document care.
• Licensed driver with a valid vehicle and insurance.

Responsibilities

• Provide personal care and assist with daily activities as assigned.
• Maintain patient safety and use equipment properly (e.g., steps, rails, O2).
• Assists with bathing, grooming, oral care, and changing bed linens.
• Assist with feeding and monitor patient intake.
• Assist with dressing, transfers, and ambulation per plan.
• Support with light housekeeping and laundry tasks as needed.
• Document observations and care provided in EMR as required.
• Communicate with RN Case Manager and family regarding patient status.
