Job summary

Affinity Health Management is seeking a hospice aide to work under an RN, providing personal care and support per the patient’s plan of care. You will report observations, assist with daily activities, and help maintain safety and comfort for patients in a home-based or facility setting.

The role requires a state-approved CNA course, NAC certification, CPR, and a patient-centered approach. Prior hospice experience is a plus, with documentation in EMR and a valid driver’s license preferred.

Qualifications

• Completion of a state approved certified nursing assistant course.
• NAC certification.
• Current Healthcare Provider CPR certification.
• Current Food Handlers Permit.
• High school diploma or GED.

Responsibilities

• Assist with personal care and activities of daily living as assigned.
• Provide compassionate communication with patients and families.
• Ensure patient safety and proper use of equipment.
• Report observations and changes to the RN Case Manager.
• Maintain a clean patient living area and assist with light homemaker tasks.
• Document care in the EMR as required.
