Job summary

Affinity Hospice Management LLC is seeking a dedicated Hospice Aide to provide compassionate personal care under RN supervision. Responsibilities include assisting with daily living activities, reporting changes to the RN Case Manager, and ensuring safety and comfort for patients in their homes or care settings.

The role requires CNA/NAC certification, CPR, a valid driver’s license, and the ability to document care in EMR systems. Prior hospice or long-term care experience is preferred.

Qualifications

• Ability to read and follow written instructions and document care provided.
• Basic computer skills to document in EMR systems.
• Licensed driver with insured vehicle.
• CNA/NAC certification and CPR are required.
• High school diploma or GED required.

Responsibilities

• Perform personal care tasks as assigned in the patient’s plan of care.
• Report and document observations to the RN Case Manager.
• Assist with activities of daily living and mobility as directed.
• Support safety and comfort, using equipment properly.
• Provide meals or snacks as delegated and observe dietary restrictions.
• Maintain clean, safe patient living area and proper infection control.
• Communicate with patient, family, and care team with confidentiality.
