Job summary

Affinity Hospice Management LLC is seeking a hospice aide to work under the supervision of a registered nurse to support patient care according to the plan of care. The role emphasizes comfort, safety, and timely reporting of observations to the RN Case Manager.

The aide will perform personal care tasks, assist with activities of daily living, observe skin condition, and ensure proper documentation while maintaining confidentiality and compassionate communication with patients and families.

Qualifications

• Completion of a state approved certified nursing assistant course or equivalent training with current license.
• High school diploma or GED.
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy and comfort care for terminally ill.
• Prior home health/hospice experience preferred.
• Basic computer skills to document in electronic medical records (EMR).
• At least 18 years of age.
• Licensed driver with insured vehicle in good condition.
• Must be fully vaccinated.

Responsibilities

• Perform personal care tasks and attend to patient requests promptly.
• Communicate with patients and families with sensitivity and confidentiality.
• Ensure safety and proper use of equipment.
• Provide baths, grooming, oral hygiene, and linen changes.
• Assist in dressing, feeding, and turning/positioning.
• Assist with self-administered medications as allowed by state regulations.
• Report pain or changes to RN Case Manager.
• Support patient mobility and range of motion as directed.
• Keep living area clean and organized; adhere to care procedures.
