Affinity Health Management is seeking a Hospice Aide to work under the supervision of a registered nurse. You will perform personal care and comfort tasks per the patient’s plan of care, and report observations to the RN Case Manager.

The role emphasizes safety, confidentiality, and compassionate terminally ill care. Responsibilities include helping with baths, grooming, feeding, transfers, and light homemaker tasks while maintaining documentation in EMR and adhering to hospice philosophy.

Qualificações

• Completion of a state approved certified nursing assistant course or equivalent training.
• Nursing Assistant Certified (NAC)
• Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer)
• Current Food Handlers Permit
• High school diploma or General Equivalency Degree (GED)
• Ability to read and follow written instructions and document care given.
• Understands hospice philosophy, comfortable providing specialized care to the terminally ill.
• Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred
• Basic computer skills to document in electronic medical records (EMR)
• At least 18 years of age.
• Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.

Responsabilidades

• Perform personal care tasks as assigned and respond promptly to patient requests.
• Communicate with patients and families in an ethical, respectful, and confidential manner.
• Ensure safety and proper use of equipment and assist devices.
• Provide baths, grooming, oral hygiene, shampoos and bed linen changes.
• Assist in dressing, undressing, and feeding as assigned.
• Assist with self-administered medications as allowed by state regulations.
• Support mobility, transfers, and range-of-motion exercises as directed by the nurse.
• Keep patient areas clean and report changes to the RN Case Manager.
• Document observations and care performed for EMR records.
• Provide respite care to patients/families when on-site, as appropriate.
