Affinity Health Management is seeking a hospice aide to support patients under the supervision of an RN, providing personal care and comfort per the patient's plan of care. You will report observations, assist with activities of daily living, and maintain a clean living area while upholding confidentiality and quality standards.

Responsibilities include safety, mobility assistance, hygiene, feeding support, and documentation in EMR.

Qualifikationen

• Completion of state-approved CNA course or equivalent training.
• Current NAC certification or equivalent CNA credential.
• Licensed driver with insured vehicle as required by organization.

Aufgaben

• Provide personal care and attend to patient requests promptly.
• Communicate with patient and family with confidentiality and respect.
• Ensure safety and proper use of equipment and supplies.
• Assist with bathing, grooming, oral care, and linen changes.
• Assist in dressing, feeding, and mobility as assigned.
• Report changes to RN Case Manager and document care provided.
• Maintain clean, orderly patient living area and adhere to procedures.
• Document care in EMR and participate in quality improvement efforts.

Certified Nursing Assistant

CPR certification

Home health/hospice experience

Reading and following instructions

Basic computer skills

Driver license
