Job summary

Affinity Hospice Management LLC is seeking a dedicated hospice aide to work under the supervision of a registered nurse, performing personal care and tasks to meet each patient’s plan of care and promote comfort. The role involves documenting care, reporting observations, assisting with activities of daily living, maintaining safety, and supporting families.

A CNA/HC aide background and CPR are typically required, with prior hospice/home health experience preferred.

Qualifications

• Completion of state-approved CNA course or equivalent.
• NAC certification.
• CPR certification current; hands-on verification required.
• Food Handler Permit.
• High school diploma or GED.
• Ability to read and follow written instructions and document care.
• Understands hospice philosophy and comfortable caring for terminally ill.
• 1 year prior hospice/home health experience preferred.
• Basic computer skills to document in EMR.
• At least 18 years of age.
• Licensed driver with insured vehicle.

Responsibilities

• Perform personal care and attend to patient requests promptly.
• Provide positive communication with patients and families while maintaining confidentiality.
• Ensure safety and use equipment properly.
• Assist with baths, oral hygiene, hair care, and linen changes.
• Assist with dressing and undressing.
• Assist in feeding; avoid homemade foods from patients.
• Monitor skin integrity and prevent tissue breakdown.
• Report pain or changes to RN Case Manager.
• Assist with ambulation and prescribed exercises.
• Support ROM and positioning as directed.
• Assist with medication reminders per state rules.
• Assist with bedpans, urinals, light housekeeping.
• Perform ROM and simple procedures as competency allows.
• Provide respite for patient/family when on-site.
• Keep living area clean and orderly.
• Follow documentation and care procedures.
• Participate in quality improvement activities.
• Respect patient and family environment and needs.
• Report non-compliance with Dept of Health within 14 days.
• Report suspected abuse or neglect immediately when warranted.
