Job summary

Affinity Hospice Management LLC is seeking a licensed Registered Nurse to plan, organize and direct hospice care in the Washington state area. The nurse will work with patients and families in home and community settings, focusing on palliative nursing actions and care planning.

The role requires comprehensive nursing assessments, administration of prescribed medications, and collaboration with an interdisciplinary team to meet evolving patient needs and ensure quality care and safety.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse licensed in good standing in Washington state.
• Minimum of 1 year of professional nursing experience in last 3 years; or BSN with at least 2 years in public health/home care/hospice preferred.
• Management experience not required; responsible for supervising hospice aides.
• Offer contingent upon satisfactory references and criminal background checks.

Responsibilities

• Performs initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Delivers professional nursing care using all elements of the nursing process.
• Develops and revises care plans with patient/family involvement.
• Administers medications and treatments per physician's plan of care.
• Provides patient and family education and counseling on nursing needs.
• Maintains accurate clinical notes and communicates with physicians and care team.
• Actively participates in on-call duties and coordinates equipment and services.
• Ensures safe environment and continuity of care with the interdisciplinary team.
