Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize, and direct hospice care in home and community settings. The RN will perform comprehensive assessments, develop care plans, and coordinate interdisciplinary services to support patients and families.

The role requires licensure and relevant nursing experience, with emphasis on community health education and palliative care. On-site home visits and collaboration with the clinical team are essential.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Minimum 1 year of nursing experience in the last 3 years; or a BSN with RN license and minimum 2 years’ experience, at least one in public health, home care, or hospice.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments.
• Develop, implement and revise care plans; initiate appropriate revisions.
• Administer medications and treatments as prescribed.
• Counsel patients and families; provide health education.
• Coordinate patient care as Case Manager when assigned; participate in interdisciplinary teams.
