Job summary

Affinity Health Management seeks a Registered Nurse to plan, organize and direct hospice care in home and community settings. The nurse will build on community resources to meet needs of patients and families, with emphasis on education and supportive care.

The role requires comprehensive assessments, care planning, medication administration, and collaboration with an interdisciplinary team. On-call duties and travel within local area apply.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• 1 year of experience as a professional nurse in last 3 years, or a BSN with 2 years’ experience in public health/home care/hospice preferred.
• Must be a licensed driver with insured vehicle.
• Possesses CPR certification.

Responsibilities

• Completes comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Educates patient and family on self-care and health management.
• Acts as Case Manager for assigned caseload when required.
• Participates in on-call duties and ensures necessary equipment is available.
• Maintains accurate clinical notes and communicates with physicians and care team.
