Job summary

Affinity Health Management in the United States seeks a registered nurse to plan, organize and direct hospice care, leveraging community health experience to serve patients in their homes and communities.

You will perform comprehensive assessments, develop and revise care plans, administer medications, and educate patients and families while coordinating with an interdisciplinary team to provide compassionate, high-quality palliative care.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• Minimum 1 year of professional nursing experience within the last three years; or a baccalaureate degree in nursing with at least two years' experience in public health, home care, or hospice preferred.
• Management experience not required; may supervise hospice aides.
• Employment contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication, problem solving and basic math skills.
• Must be licensed driver with insured vehicle in good working order and CPR certification.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plan of care based on patient status and needs.
• Implement plans of care and administer medications and treatments as prescribed.
• Counsel patients and families and provide health instructions.
• Act as Case Manager for assigned caseload and coordinate care with the interdisciplinary team.
