Job summary

Affinity Hospice Management LLC in Washington state is seeking a registered nurse to plan, organize and direct hospice care in home and community settings, applying strong nursing skills and a focus on patient- and family-centered care.

You will perform comprehensive assessments, develop and revise plans of care, administer medications, educate families, coordinate with the interdisciplinary team, and participate in on-call duties to ensure continuous quality care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in good standing.
• Minimum 1 year of professional nurse experience within the last 3 years, or a BSN with 2+ years in public health/home care/hospice preferred.
• Management experience not required; may supervise hospice aides.
• Background checks and references required upon offer.
• Excellent observation, verbal and written communication skills; problem solving; nursing skills per competency checklist.
• Must be able to lift and transfer patients and have a valid driver’s license with insured vehicle.
• CPR certification (hands-on verification) required.

Responsibilities

• Completes initial and ongoing comprehensive assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes and revises plan of care as patient status changes.
• Initiates plan of care and revisions as needed.
• Develops care plans with patient and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care education to patient and family.
• Performs on-call duties and ensures equipment/ services are available.
• Acts as Case Manager when assigned and coordinates patient care.
