Job summary

Affinity Hospice Management LLC seeks a registered nurse to plan, organize and direct hospice care in home and community settings. The RN will conduct initial, comprehensive assessments with patients and families, develop plans of care, and coordinate services to meet individual needs.

The responsibilities include administering medications, teaching self-care to patients and families, and coordinating with physicians and interdisciplinary teams.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Experience: 1 year in last 3 years or 2 years in public health/home care/hospice preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessment of patient and family.
• Develop and revise the plan of care with patient and family input.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in nursing needs and self-care.
• Coordinate patient care as Case Manager when assigned.
