Job summary

Affinity Health Management is seeking a Registered Nurse to lead hospice care within home and community settings. You will perform comprehensive patient assessments, develop and adjust care plans, and collaborate with families to ensure comfort and symptom control.

The role emphasizes nursing skills, patient and family education, and coordination with an interdisciplinary team to provide quality palliative care in the community.

Qualifications

  • Graduate of an accredited school of nursing.
  • Registered Nurse with a license in good standing to practice.
  • Minimum of one year of professional nursing experience within the last three years; or a BSN with two years’ experience, at least one in public health, home care, or hospice preferred.
  • Management experience not required. Responsible for supervising hospice aides.
  • Offer contingent upon satisfactory references and criminal background checks by regulation.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Prolonged or considerable walking or standing; able to lift, position and/or transfer patients; able to lift supplies and equipment; reaching, bending, kneeling.
  • Must be a licensed driver with an automobile that is insured and in good working order.
  • Possesses CPR certification (hands-on preferred).

Responsibilities

  • Complete initial, comprehensive patient and family assessment to determine hospice needs.
  • Provide professional nursing care using the nursing process.
  • Assess and evaluate patient status; write and initiate plan of care; revise plan as needed.
  • Initiate and revise the plan of care as patient needs change.
  • Use health assessment data to determine nursing diagnosis.
  • Develop a care plan with goals based on nursing diagnosis and involve patient and family.
  • Initiate preventive and rehabilitative nursing procedures; administer medications as prescribed.
  • Counsel patient and family in meeting nursing and related needs.
  • Provide health care instructions per assessment and plan.
  • Assist patient with activities of daily living; support self-sufficiency and comfort.
  • Act as Case Manager when assigned to coordinate patient care.
  • Complete, maintain and submit accurate clinical notes; record pain/symptom changes.
  • Communicate with physician regarding patient needs; obtain orders.
  • Coordinate care with community health professionals.
  • Report non-compliance to Dept. of Health within 14 days.
  • Report suspected abuse/neglect to authorities immediately.
  • Teach patient and family self-care; provide medication/diet instructions; collaborate with team.
  • Provide and maintain a safe environment for the patient.
  • Assist with continuity of care with family and hospice team.
  • Work with family/caregiver and team to meet emotional needs.
  • Attend interdisciplinary group meetings.
  • Participate in on-call duties as defined by policy.
  • Arrange equipment and services as needed.
  • Supervise ancillary staff and delegate responsibilities as required.
  • Pursue personal growth with continuing education and inservice activities.
  • Fulfill every assigned case.
