Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize, and direct hospice care in home settings, using community resources to meet patient and family needs.

The RN will complete comprehensive assessments, develop and revise care plans, administer treatments, educate patients and families, and collaborate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing.
• Minimum of 1 year of professional nursing experience within the last 3 years; or a BSN with at least 2 years of experience in public health, home care, or hospice.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patient status by writing and initiating plan of care.
• Revises the plan of care as necessary as patient status changes.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Implements preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Teaches patient and family self-care techniques and works with the interdisciplinary group.
